Pet Vaccination & Liability Acknowledgment (Required only for participants bringing pets) 

☐ I confirm that any pet(s) I bring to the event are current on all required vaccinations, including rabies. I further confirm that any pet(s) I bring to the event are socialized and have not demonstrated aggressive behavior toward adults, children or other animals.

☐ I understand and agree that participation in this event is at my own risk.  I hereby assume full responsibility for any risk of bodily injury, death, or property damage arising out of or related to my participation in the event. I will not hold Spartanburg County or its employees, the Pet Resource Center, event organizers, sponsors, volunteers, or other participants liable for any injury or illness to myself, my pet(s), my child(ren), or others that may occur before, during, or after the event. 

I, THE UNDERSIGNED, HAVE READ THIS WAIVER OF LIABILITY, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
