By signing, I hereby attest that the information above is legal and correct and agree to allow the Area Office on Aging of Northwestern Ohio, Inc., and the Northwest Ohio Area Office on Aging Foundation to identify and utilize my email, any photographic and/or video images in which I appear for any and all applications that they deem suitable, including but not limited to, newsletters, promotional presentations and other printed or videographic material. I understand that my participation in media interview(s) and/or photograph(s) is at my discretion and is voluntary. I understand the news media are not subject to the confidential requirements of the Area Office on Aging of Northwestern Ohio, Inc., the Senior Citizens Committee, and the Northwest Ohio Area Office on Aging Foundation and I waive all ownership rights to these images and I understand that I will receive no financial compensation for the use of these images or for participating in the media interview(s).
