There are 2 options for this event, you can sign up for one of the 25 spots to receive a FREE Sports Physical and participate in the rest of the sessions (Soccer Skills Clinic and Health & Wellness) OR you can sign up for JUST the Soccer Skills Clinic and Health & Wellness Sessions.

Sports Physical Information

This event is FREE and open to middle and high school students only and the sports physicals are good for the 2022-2023 school year.

After signing up for an appointment, please make sure that page 1 and page 2 of the VHSL Athletic Participation/Parental Consent/Physical Examination Form (which can be downloaded here) are completed prior to the youth’s appointment.  You will need to bring the form to the youth’s appointment as well so the physician can complete it.  If a student is getting a physical, they are required to be in Gym Clothes!

Physical Forms can be picked up at most schools, Jubilee Family Development Center and Boys & Girls Club of Lynchburg
